Forms linked below will help to expedite and add efficiency to common requests for the Paramount Unified School District Employees.
BUSINESS SERVICES
  • Claim Form - Form for filing a claim against Paramount USD for vehicle damage, property damage or personal injury.  Please contact the Business Services office (562-602-6025) with any questions.
  • Facility Modification Form - A Facility Modification Form is to be completed for any changes and/or modifications you plan on making at your school or facility.  The form shall be completed and approved before items are purchased to ensure that the modifications can be made.  Please contact the Facilities Department (562-602-8088) with any questions.
EDUCATIONAL SERVICES
  • Request for Site Event and/or Flyer Distribution - This form is to be used by school administrators seeking approval for an event and/or flyer distribution at their school site.  If you have any questions prior to submitting the request, please contact Ed Services at ext. 6017.
  • Notification of Contracted Services - Form for advance notification to Ed Services for an upcoming contracted service or consultant request.
FISCAL SERVICES
  • Expense Claim Form - Form for Expense Claim(s) tied to an approved PAA Form. Please have the PAA # ready.
  • Mileage Claim Form - This form is to be used for monthly mileage claims reimbursements.  Please contact your school/department administrative assistant for the correct account string for this reimbursement type.
  • PAA/CAR Form - Form for Professional Activity Attendance requests and Cash Advance Requests associated with the Professional Activity. Please contact your school/department administrative assistant for the correct account string for this reimbursement type.
  • Payroll Information Request - Form for information requests such as W-2 Form(s) and/or Pay Stub(s).
HUMAN RESOURCES
  • District Leave Form - Form for all District Leave Requests such as Vacation, Personal Necessity, Bereavement and Confirmation of Ill Time Use. Please contact your school/department administrative assistant with any questions.
  • Employee Resignation Form - Form to be completed by employees who are resigning from PUSD.
  • Warrant Designation Form - Form used to update warrant recipient for PUSD employees.
PRINT SHOP
  • Banner & Poster Order Form - Form used to submit orders for banners and/or posters from the Print Shop.  Prior to submitting your order, make sure you have obtained the account string from your administrative assistant for billing.  If you need to submit a regular print shop order, please click here to redirect you to the Print Shop Online website.
PURCHASING
  • Conference, Hotel and Travel Form - The Conference, Hotel & Travel Request Form (CHT Form) is required by District staff to request payment for conference attendance and associated travel arrangements. Submission of this form requires an approved Professional Activity Attendance Request (PAAR), and the corresponding PAAR number is mandatory for completion of the CHT Form. Additionally, valid purchase requisition number(s) are required as well.
RESEARCH, ASSESSMENT & STUDENT INFORMATION (RASI)
  • District Course Form - Form used for sites to request: new courses, changes to existing courses and activation/inactivation of courses.  If you have any questions, please contact the SIS department at extension 6036.
  • Network/Synergy Access Request Form - Form used for sites to request for the following:  Network/E-mail accounts, Synergy account and access, & CALPADS access.  If you have any questions, please contact the RASI department at extension 6003.
  • Request for Access (Non Employees) - Form used for vendors or authorized individuals (non employees) to request access to the network and/or systems.  If you have any questions, please contact the RASI department at extension 6003.   
  • ParentVUE Request Form - Form filled out and submitted by parents in order to gain access to ParentVUE.  If you have any questions, please contact the RASI department at extension 6003.
  • Systems Codes Request Form - Form used for changes to codes within the Synergy software platform.  If you have any questions, please contact the SIS department at extension 6036.
SAFETY & SECURITY
  • Lost Key Form - Form for staff to report LOST or STOLEN key(s).  Before completing this form:
    • (1) Contact the local police department where the key(s) were lost/stolen.  You will need to obtain a service or tag number for this call;
    • (2) Contact the Safety & Security Department at (562) 602-8088 and verbally report lost/stolen key(s).
  • Security Incident Report - Form used to report an incident.  Incidents include theft, vandalism, trespassing, etc. that occurred on District property.  Please contact the Safety & Security department (562-602-8088) if you have any questions.
  • Staff ID/Badge Request Form - Form for request of Staff ID/Badge. This form to be used for ID requests for new staff or for the replacement of an ID that has been lost, damaged or simply to update information for changes in name, title, job site, etc. Please contact the Safety & Security department (562-602-8088) if you have any questions.
SPECIAL EDUCATION
  • BIS Referral Form - Please use this form when referring either General Education and Special Education students for Behavior Intervention Supports. Feel free to reach out to the BIS team with any questions or needing support in completing the referral.
TRANSPORTATION
  • Transportation Request Form (Daily) - Form for daily student transportation requests.  Digital form currently suspended, please contact the Special Education department (ext. 6033) to obtain instructions.
  • Transportation Request Form (Field Trips) - Form for field trip requests.  Please have all field trip information ready prior to filling out this form inlcuding destination details, total number of students and adults, account string, etc.  Please click here to access an informative step by step guide on filling out this form.
